SECTION III THE ANTERIOR VESSELS OF THE UPPER HALF OF THE LEG General Considerations.--There would be no need to describe the ways for giving free access to the anterior vascular trunks at the upper part of the leg were it not for the fact that surgeons still lose much valuable time in the attempt to find the famous interstice that separates the tibialis anticus from the extensor communis digitorum. Does this imply that this difficulty is great? By no means whatsoever. But it unquestionably is quite useless to battle with an obstacle when one can easily get round it otherwise. Now, in the case under consideration nothing can be easier, because all that is necessary is to look directly for the detachable space, at the bottom of which the vessels lie. This space is found at the point where the tibialis anticus and extensor communis are no longer adherent to one another and are far enough apart for finding their interstice at once. In presence of a haemorrhage many surgeons instinctively prolong their incision downward, and, consequently, at once enter the interstice. The procedure we are about to describe must certainly have been done hundreds of times, although surgeons have not regarded it as a technical innovation for the very simple reason that this procedure appeared to be perfectly natural. Besides the much greater facility offered for exposing the vessels by carrying the incision further down the leg than usual it is also advantageous in that it exposes them over a really appreciable extent --they are quite deeply seated in this portion of the leg--and also allows the outward retraction of the extensors to a far greater degree.
